The Healthy Child Programme (0-5 and 5-19) aims to give every child the best start in life and reduce inequalities throughout life. The programme is part of the Public Health Nursing (PHN) Service commissioned by Local Authorities. Packages of care are provided by Health Visitors for children aged 0-5 and by School Nurses for children and young people aged 5-19.
Hertfordshire Community NHS Trust successfully tendered to continue providing the PHN service in Hertfordshire, and were tasked with;
- Delivering the same quality of service within a progressively reducing financial envelope over the contract lifetime
- Meet challenging Key Performance Indicators (KPIs)
- Work closely with new Family Support Service (FSS) Partners in Hertfordshire to deliver an integrated service, branded the ‘Family Centre Service’ (FCS).
BearingPoint worked closely with Hertfordshire Community NHS Trust to support these objectives.
BearingPoint designed three phases to support with this transformation. Firstly, a behaviour change solution to drive bottom up change. Secondly, empowering management with service delivery data. Finally, releasing more time to care, with a strong focus on transforming services for 0-5-year olds. Throughout the process of transformation, BearingPoint continuously engaged with frontline staff to co-design solutions, build staff confidence, develop strong improvement and demonstrate value for money.
With this three phase transformation process taking place alongside business as usual – with no break in service to redesign delivery – BearingPoint was able to deliver significant results, supporting with the overall objectives from the Local Authority.
